There are two options for obtaining a copy of the registration form.

  1. Download the form in MS Office document format: registration.doc
  2. Request a paper copy from [email protected] (please include your postal address).

CONFERENCE FEES

The conference fee includes a copy of the proceedings, lunches, morning and afternoon teas. Listed below are the basic conference fees. Please see the registration form for full information.
